Both Dr Koto and Ruri no shima were filmed in Okinawa (both take place in Okinawa as well). But I think they are different islands.

The 2004 special will most likely be subbed. I'm not sure what the other special is...

I'm just referencing the link you made in your original post.

According to the Daddicts drama wiki there was one special that played at the beginning of 2004 and another special at the END of 2004.

P.S.  Your excuse that they're supposed to have bad/grey teeth because they're on a remote island doesn't seem to explain why so many japanese actors/actresses have them.. even when they're playing someone NOT on an island.
